Extra Care Services
?An Estate Manager, employed by SCHA, is based at Phoenix House and oversees the day-to-day running of the scheme
?All residents of Phoenix House are provided with a care and/or support plan which is prepared in consultation with them and individually tailored to their needs
?Care and support is delivered either by a preferred domiciliary care provider based in Phoenix House which is currently Accord Care and Support ? or by a provider of a resident's own choice
?Domiciliary care provision is registered with the Care Quality Commission (CQC)

Charges and Payments

Residents pay a monthly rent, service charge and a support charge, each of which are reviewed annually. (Support could be reviewed more frequently if there is a change in circumstances.) Some of these charges are currently eligible for Housing Benefit and Supporting People subsidy.

The rent covers the accommodation, housing management and building maintenance costs. The service charge covers heating and all communal facilities expense charges as well as a proportion of the lunchtime meal cost. The support charge covers the cost of providing housing related services.

Shared Ownership purchasers pay a service charge and support charges but an element of the service charge is to cover housing management costs.

For both residents and purchasers, there may be a charge for the provision of care based on SMBC's charging policy. Otherwise, any care needed can be purchased through a private arrangement.
The exact amount each resident pays depends on their particular circumstances.

Following assessment you will receive a care and/or support plan suited to your individual needs, provided by either the on-site care and support provider or a provider of your choice
If you do not meet the criteria for care being provided by Solihull Care Trust you can purchase it from the domiciliary care team on site or from another care provider
The housing-related support service provides the on-call emergency response service; assistance to gain access to other services; such as assistance with benefit claims, or support to undertake daily living activities for example shopping or laundry; and support to organise and take part in activities
A cleaning service will ensure all the communal areas of the scheme are cleaned to a high standard. You will also be able to purchase the services of the cleaning service, or alternatively make your own arrangements

Eligibility for the scheme
The scheme is intended primarily for people aged 55 and over, or those with a disability, who live in the Solihull area or have a strong connection with the area.
To apply to rent a apartment you must have care or support needs and be willing to take part in a community care assessment which will identify your level of need
Shared ownership apartments will be available to people who meet the age/disability and locality criteria, who are suitable for the service, and who would benefit from the additional support, safety and security that this type of accommodation can offer.  

ASSESSMENT All prospective applicants for accommodation will need to complete an application form and undergo a home assessment to confirm eligibility for the scheme. Decisions on allocations will be taken by a Panel comprising representatives from SCHA, Solihull Council's Adult Social Care Team, Solihull Community Housing and the care & support provider commissioned by the Council ? Sanctuary Home Care.
